I, _____________________________, agree to facilitate a student focus group on October 23, 2001 at South Kantapple Middle School from 12:30 p.m. to 2:00 p.m. I understand that a bus will pick me up at 11:45 p.m. to take me to the site, and that I will need to stay at least 30 minutes after the completion of the session.

I agree to conduct the group in a neutral manner under the guidance of the site manager, and I agree to keep in full confidence everything student participants say during the session.
